The Vital Records Section of the Vanderburgh County Health Department started keeping records in February 1882. There are no records in Indiana before that time. The Indiana State Department of Health in Indianapolis started keeping records in October 1907.
Records can be obtained from our office, on a walk in basis, Monday through Friday (except legal Holidays), 8:00 a.m. to 4:00 p.m.
Indiana requires an applicant requesting a birth or death certificate, whether by mail or in person, be required to show identification and proof of relationship. Identification and eligibility requirements are listed below.

One form of Primary Documentation All documents MUST be current and valid
- Valid Driver’s License, Military I.D., State I.D. card, Valid Passport
- Department of Correction I.D. (issued within past 6 months)
- School I.D. with signature and photo
- Court Order (must order local health dept. to release record to person named on the record)

- The individual can complete form with proper information and identification
- Person named on record over 18 (under 18 must have letter from parent)
- Parents of person named on record (must be listed on record)
- Sibling over 18 with proof of relationship to at least one parent
- Grandparents (must be parent of a parent on the record & show proof of relationship)
- Child over 18 of person named on record with proof of relationship
- Spouse, with proof of relationship (e.g: marriage license, birth record of child, insurance card)
- Court Appointed Legal Guardian (must provide original guardianship papers with seal)
- Attorney representing person named on record (must have I.D. for self and I.D. with permission letter from person named on record.)
- Caseworker from Division of Family & Children with court appointed guardianship paper (must show State Issued I.D.)
- Law Enforcement personnel with Appointing Authroity Issued I.D. and court order
- Adult child or grandchild (proof of relationship)

If you do not have primary documentation, two forms of Secondary Documentation are required
- Bankcard with signature (not credit cards) or personal check with current information
- Club membership card with signature or photo
- Employment I.D. with signature, photo, date of employment or employee address
- Expired driver’s license (not more than 6 months)
- Gun permit with signature
- Letter from BMV or Social Security Administration that shows individual's name and date of birth
- Previous year’s tax return with signature and social security number
- Probation documents or statement from Probation Officer on letterhead, including person’s name and date of birth
- Signed Leases or loan agreements
- Social Security card
- Valid Indiana Professional License
- Vehicle Registration with signature
- Voter Registration with signature
- TANF (Welfare), Food Stamp or WIC I.D. cards

Certified copy of birth certificates effective May 1, 2006 - $10.00
Certified copy of death certificates effective July 1, 2007 - $11.75
